Can it issue a refund on its own?

No. It can find the order, explain the returns policy and open the record, but the refund itself is approved by someone on your team.

Does it need access to our shop?

It reads order and product data from the tools you already use and writes back into them. The data stays in your shop; nothing is moved into a system you cannot see.
